Monday - Friday, 8:00 AM - 5:00 PM

Assignment Details

Contract length: 13 weeks, Guaranteed hours: 40

Requirements

• Active RN license in the state
• BSN required
• 2+ years of acute care Case Manager experience required

Responsibilities

• Coordinate patient care and services across the continuum of care
• Manage and facilitate patient transitions from one care setting to another
• Assess patient needs and develop comprehensive care plans
• Collaborate with healthcare team members to ensure quality care
• Monitor patient progress and adjust care plans as necessary
