Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with the patient's needs, goals, and physician direction. This position is responsible for the delegation and direction of discipline-specific therapy services, ensuring the design of functional treatment programs that allow for an effective return to community activities. The therapist serves as an educator for patients, families, students, and coworkers, while adhering to professional standards and codes of conduct.

Responsibilities

• Completes comprehensive evaluations and develops appropriate treatment plans focused on patient progress.
• Ensures clear, concise, and timely medical record documentation, in compliance with state and federal guidelines.
• Participates in student programs, providing supervision and a positive learning environment.
• Actively pursues professional development through continuing education and self-study.
• Maintains adherence to discipline-specific State Practice Acts and completes all state-mandated continuing education requirements.
• Demonstrates awareness of organizational performance improvement initiatives and offers feedback for enhancement.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 1+ year of outpatient clinical experience.
• Current CPR certification.
• Active and applicable state license for Physical Therapy.

Schedule

This is a full-time position, functioning on a 5x8.0 hour shift.
