Key Responsibilities:

  • Deliver physical therapy services to patients in their homes, including assessments, treatment plans, and interventions tailored to each individual’s needs.
  • Evaluate the home environment and recommend modifications or resources to improve safety and mobility.
  • Monitor patient progress and communicate updates or changes in condition to physicians.
  • Collaborate with caregivers, nurses, and other healthcare professionals to ensure coordinated, comprehensive care.
  • Refer patients to additional home health services when appropriate.
  • Participate in discharge planning and ensure continuity of care.
  • Supervise Physical Therapy Assistants in accordance with company policies and state regulations.

Qualifications:

  • Bachelor’s or Master’s degree in Physical Therapy from an accredited program.
  • Current Physical Therapy license in the state of Pennsylvania.
  • Minimum of 2 years of experience as a Physical Therapist; home health or community-based care experience preferred.
  • Valid CPR certification.
  • Reliable transportation and a valid driver’s license.
