Nurse Practitioner / Hospice Case Manager




Constellation Hospice in Philadelphia is looking for a patient centric NP/Hospice Case Manager for a caseload of 13-15 patients.  You will practice at the top of your licensure and provide face-to-face visits for that caseload as required! 




Requirements:



  • Current Nurse Practitioner NP/APRN Pennsylvania state license

  • Prescriptive authority.

  • Active Pennsylvania DEA.

  • BLS & ACLS.

  • Valid Driver's License.

  • Reliable vehicle. 

  • Ability to apply both hospice & palliative (consultative) concepts and philosophies to patient/family care required.

  • Experience with hospice care, palliative care and/or geriatric care. 

  • Excellent communication and computer skills.

  • Willing to travel to see patients. 


 


Responsibilities:



  • Manage a caseload of 13-15 patients. 

  • Complete Plans of Care and IDG for these patients.

  • Rotate 1 week on call every 6 weeks but truly act as an NP.

  • Be the attending if/when needed.

  • Works with Business Development, Collaborating Physician (Palliative Oversight), Hospice Medical Director and the Hospice Interdisciplinary Team (IDT) to ensure the provision of physical, emotional, spiritual, social and bereavement care to adult, geriatric, pediatric, and neonatal Hospice Patients, and their families.

  • Assists in determining the patient's medical eligibility for both Hospice & Palliative (consultative) services by visiting the patient and may be asked to perform a face-to-face encounter 30 days prior to the 180-day recertification and then again, every 60 days thereafter.

  • Works in cooperation with the IDT to develop and update care plans for those patients the APRN has assessed for recertification (Face to Face) or symptom management visits.

  • Acts as a resource to attending physicians, staff, hospice & palliative clients and families as requested regarding pain and other symptom management to assure the patient/family needs are met.

  • Communicates effectively and tactfully with clients, recognizing their age, cultural diversity, needs, abilities and physical condition.

  • Will utilize computer-based documentation system and submit all required clinical documentation, time records and other required information in required time frame.
