Attentive Care Service Agency is currently seeking to hire Licensed Practical Nurses (LPN)




Job Description




LPN to conduct home visits position entails visiting patients in their home for skilled nursing assessment, wound care, medication management, health care management, etc. The position requires a person with a reliable vehicle to visit a patient's home to perform skilled nursing visits. Mileage reimbursement is not covered by the Agency. Nursing visits take approximately 1 hour in length for each visit. The qualified candidate will be knowledgeable with documenting care provided during visits by body systems. Accurate, thorough, precise, objective documentation is a requirement for this position. This position is PRN and is not full time.

Qualifications





  • Successfully complete an approved nursing program and obtain an Associate’s degree or higher

  • Successfully pass the NCLEX exam

  • Current unrestricted license verified through the state Board of Nursing

  • One year experience as an LPN

  • Provide and maintain a professional nursing license in accordance with state and federal regulations

  • Familiarity with the duties of an LPN

  • Provide current documentation to HR for reference checks, background checks, residency checks, etc.

  • Pass a state, federal, ChildLine background check




Responsibilities





  • Providing services in accordance with Agency policies and procedures

  • Prepare clinical and progress notes

  • Helping patients bathe, get dressed, go to the bathroom, and eat

  • Monitoring patients’ overall health during every visit

  • Changing bandages, wound dressings, and catheters

  • Setting up exercise programs for patients

  • Verifying patients are taking the correct dosages of medications

  • Assist patients in learning appropriate self-care techniques, as applicable

  • Participate in in-service programs




Salary 



  • The pay for this position ranges from $30-$35 per hour. Pay rates are determined based on experience, distance, and/or availability.




Visits: Visits can be completed mornings, afternoons, evenings, and nights


Please note that this is a 100% per diem position. Entry-level experience welcomed.
