Position Summary
PMHCC Case Management is a unit dedicated to providing recovery support services to individuals as they journey through their recovery process from the disease of addiction and other life challenges with the goal of achieving and maintaining long-term recovery. As case managers work with individuals in their recovery process, individuals are empowered to identify their strengths and talents and focus on the areas of need that he or she is willing to address at specific points in time. This collaborative approach is built upon the idea that all individuals, regardless of their challenges and walks of life, are people first.
The Enhanced Case Manager (ECM) will coordinate formal and informal treatment and provide a strengths-based approach to working with individuals seeking ongoing support needed to facilitate long-term recovery and management of Permanent Support Housing. The ECM will work in a team model to provide intensive support to those individuals with significant reoccurring substance abuse and mental health challenges. The ECM’s takes the lead in addiction and recovery and one (1) of five (5) areas of specialty: vocational/employment services, housing, mental health challenges, basic needs/life skills or forensics. The ECM’s will support the individual holistically with addressing the challenges to maintaining long-term recovery; while identifying, developing, implementing recovery-oriented goals and monitoring the service recovery plan. Also, ensuring the mission, goals and philosophy of PMHCC Case Management are operationalized within the team.
Duties and Responsibilities:
Skills Required:
Must have excellent verbal/written communication skills, be detail-oriented with exceptional time management and organizational skills. Must possess a strong ability to engage with individuals experiencing significant substance abuse and mental health challenges. Must possess exceptional critical thinking skills and the ability to work independently and interdependently within a team environment. Must be strong in advocacy and navigating complex systems. Must be proficient in using Microsoft Office, databases, web-based browsers and basic typing.
Must display a mastery of the following case management core competencies:
Education and Experience:
Bachelor degree required with major course work in Sociology, Social Work, Psychology, Gerontology, Anthropology, Political Science, History, Public Health Criminal Justice, Theology, Nursing, Counseling or Education. Master’s Degree preferred in Social work, Psychology, Counseling, or Human Services, strongly desired. Experience with working in a fee-for-service environment is preferred and candidates certified in addictions counseling are strongly desired. Expertise in one (1) or more of the following required: substance abuse, criminal justice system, educational/vocational, basic needs/life skills and housing.
Physical Demands:
Sitting for a minimum of one (1) hour and able to operate a motor vehicle. Ability to look at a computer monitor for a minimum of one (1) hour. Ability to use a keyboard or similar computer peripheral for typing.
Essential Functions:
Must pass FBI, PA Criminal and Child Abuse background checks. Must have a valid driver’s license and an acceptable driving record when operating a company/personal vehicle during work hours for work-related activities. Permission to operate a vehicle as part of your job duties will be contingent on your insurability as a driver consistent with PMHCC, Inc. policy. Must have the ability to physically perform the duties and able to work in the environmental conditions required of a position in this class.
