Position Summary
The ELECT Case Manager is responsible for the following providing school-based case management services, homes visits, ongoing assessment, the development and/or review of service plans, referral connection and follow-up, advocacy, data collection and data entry as well as submission of programmatic & statistical reports. This position is primarily located at School District of Philadelphia schools they are assigned to provide case management services. This position requires a sincere interest in working with and supporting teen parents, their children and their overall health and development.
The ELECT Case Manager is required to facilitate and maintain contacts and relationships with fellow ELECT Program team members, as well as other Congreso departments and programs to allow for quick referral of services outside of this program. Additionally, this position is required to maintain good relationships with the assigned staff at the schools they their assigned. This position should be knowledgeable of external resources that clients can be referred to including community resources at other organizations. This position has access to sensitive Congreso information and is expected to handle such information with integrity and professionalism. This position has regular contact with members of the community and is expected to represent Congreso in a professional manner.
This position reports to the ELECT Program Manager. This is an ONSITE position.

Minimum Experience, Education, and Qualifications
Minimum Education:
High school diploma or GED/HISET required.
Bachelor’s degree in Social Work, Psychology, Human Services, Education or related field preferred.

Valid driver’s license required.
Mandated reporter certification required, can be obtained upon hire.
