Case Manager

CUA Cares Center

Statement of General Purpose

The CUA case manager provides on-going service and support to assigned children/families ensuring the development and implementation of comprehensive deliverplans based on team assessments of client's needs. The case manager will also assist families in developing parenting practices and skills which reduce safety risks while enhancing abilities to provide a nurturing environment for children

 

Examples of Essential Functions and Responsibilities

  • Conducts intakes and provides assessments to determine the needs of the families.
  • Provides home, school and community visits to assess needs and assists in accessing resources according to policies, procedures, practices and County, State regulations.
  • Develops a comprehensive service plan, as expected via policies and procedures.
  • Facilitates access to additional resources, as needed.
  • Provides team collaboration with other components of youth and family services.
  • Creates and maintains client files and completes all required documentation in a timely manner according to regulations.
  • Provides on-call services, as requested.
  • Provides written and/or oral reports to the Department of Human Services and Family court, as needed.
  • Participates in required trainings.
  • Participates in supervision.
  • Other duties as assigned.

  • Must be proficient in Microsoft Word and basic computer applications.

 

Minimum Requirements

  • Bachelor's degree iSocial Work or related field including but nolimited to sociology, psychologycounseling, criminal justice, education, divinity, or public health administration
  • Two years of working with children and/or families
  • Driver’s license with clean driving record.

 

ADA Component

  • This role requires regular hearing, talking, walking, and sitting; occasional standing, driving, and lifting up to 25 pounds. Requires regular travel.
