Who we’re looking for:

This individual is responsible for providing case management services, discharge planning, and care coordination for clients in the program. This person may handle an independent caseload and assist other personnel by ensuring proper referral of clients to all appropriate services and resources available and necessary to them. These services and resources may include medical care, benefits, education and employment, housing, recovery and prevention services, and aftercare supports.

 

Duties may include, but not be limited to providing intake services to clients; preparing reports, treatment plans, discharge/ continuing care plans, and keeping necessary records within client database and electronic health record; coordinating the activities of one or more groups of clients; and generating referrals and aftercare plans.

Work is performed in accordance with established regulations, and policies and procedures; however, this individual is expected to exercise considerable initiative and independent judgment in achieving objectives.

 

 









Responsibilities






Your responsibilities in this role may include:

  1. Treat all clients and staff with dignity and respect.
  2. Attend all team / staff meetings as required by Supervisor
  3. Role model pro-social behavior.
  4. May be responsible for conducting and documenting intake interviews, as needed.
  5. Be responsible for a caseload of clients, as identified by program requirements and in accordance with state regulations.
  6. Complete progress notes for all client-related activities within 24 hours of service delivery.
  7. Coordinate with agency and community resources to ensure thorough treatment and discharge plans.
  8. Establish and maintain positive relationships with service providers and referral sources.
  9. Assist clients with connecting to resources needed to reach personal goals during the course of their treatment and in advance of discharge.
  10. Arrange access public assistance, social security, food stamps and medical assistance appointments to ensure timely and continued receipt of benefits.
  11. Arrange medical appointments, psychiatric evaluations, and medication, when appropriate.
  12. Refer clients to appropriate vocational and educational programming.
  13. Assist clients in developing a thorough discharge plan addressing their most significant needs.
  14. Maintain contact with clients after discharge to ensure coordination with other case management/social services/ substance use disorder treatment or services.
  15. Ensure compliance with all federal, state, local and funding source guidelines.
  16. Maintain valid driver’s license.
  17. Other duties as assigned by Supervisor.

Qualifications






Qualifications for this role:

 

Education/ Training: Case Managers must have, at minimum, a bachelor’s degree in a generally recognized clinical discipline.

 

Experience:

 

Case Manager I:

Two years’ experience providing case management and/ or drug and alcohol counseling and be licensed by the Department of State or be a Certified Allied Addiction Practitioner or Certified Associate Addiction Counselor, Certified Alcohol and Drug Counselor, or Certified Advanced Alcohol and Drug Counselor at the time of hire or being working toward licensure or certification with a completion date of no more than three years from the date of hire. The “working toward” certification must be evidenced by a log maintained in the employee file that includes hours worked/hours of supervision/ required training needed to qualify for full licensure or certification as per the PA Department of State or PCB. Said log will be monitored for compliance and evidence of active pursuit of credentialing will be monitored. 

 

Case Manager II:

Three years’ experience providing case management and/ or drug and alcohol counseling and be licensed by the Department of State or be a Certified Allied Addiction Practitioner or Certified Associate Addiction Counselor, Certified Alcohol and Drug Counselor, or Certified Advanced Alcohol and Drug Counselor at the time of hire or being working toward licensure or certification with a completion date of no more than three years from the date of hire. The “working toward” certification must be evidenced by a log maintained in the employee file that includes hours worked/hours of supervision/ required training needed to qualify for full licensure or certification as per the PA Department of State or PCB. Said log will be monitored for compliance and evidence of active pursuit of credentialing will be monitored. 

 

Case Manager III:

Three years’ experience providing case management and/ or drug and alcohol counseling including at least one year supervisory or administrative experience and be licensed by the Department of State or be a Certified Allied Addiction Practitioner or Certified Associate Addiction Counselor, Certified Alcohol and Drug Counselor, or Certified Advanced Alcohol and Drug Counselor at the time of hire or being working toward licensure or certification with a completion date of no more than three years from the date of hire. The “working toward” certification must be evidenced by a log maintained in the employee file that includes hours worked/hours of supervision/ required training needed to qualify for full licensure or certification as per the PA Department of State or PCB. Said log will be monitored for compliance and evidence of active pursuit of credentialing will be monitored.

Commensurate with education, training, and/ or experience, individuals hired for Case Manager I position should demonstrate thorough knowledge and ability; individuals hired for Case Manager II position should demonstrate significant knowledge and ability; and individuals hired for Case Manager III position should demonstrate advanced knowledge and ability of the following:

  • Significant to advanced knowledge of the basic principles, philosophies, practices, and procedures of Gaudenzia’s treatment programs.
  • Significant to advanced knowledge of the biopsychosocial characteristics and maladaptive behaviors of substance use disorders and/or mental illness.
  • Significant to advanced knowledge of individual and group behavior and the effective ways of working with people to produce ongoing recovery.
  • Specialized training experience in specific population, i.e. homeless, HIV positive, etc.
  • Ability to work effectively with diverse populations of people and aid them in adjusting to their specific challenges.
  • Ability to identify the needs of clients and to gain their confidence and cooperation in assisting them to obtain their identified goals.
  • Ability to establish and maintain effective working relationships with clients, other members of staff, outside agencies, institutions and the general public.
  • Ability to initiate and maintain contacts with social service agencies, vocational programs, educational programs, etc.
  • Ability to successfully work with others from various cultures and backgrounds.
  • Ability to communicate effectively, both verbally and in writing.
  • Ability to organize a caseload and an extensive informational system.
  • Skilled at using computers, including familiarity and faculty with Microsoft Office programs like Word and Excel.
  • Must be able to have clarity of vision at 20 inches or less.
  • Must be able to ascend/ descend stairs.
  • Must be able to push/ pull.
  • Must be able to stand/ sit for extended period with reasonable breaks.
  • Must be able to reach overhead and bend/ stoop.
