Job Description

The Department of Family & Community Medicine of the Sidney Kimmel Medical College at Thomas Jefferson University is currently accepting candidates for the position of outpatient addiction medicine and primary care physician at the Sheller Bridge Program and the Project HOME Hospital to HOME partnership as well as primary care physician at the Jefferson Family Medicine Associates practice. JFMA is the flagship academic training site of Jefferson’s nationally renowned Department of Family and Community Medicine. JFMA serves a highly diverse patient population of approximately 30,000 people with an unflagging commitment to excellence and equity across the full care spectrum of family medicine. JFMA’s care teams include 27 faculty physicians, 4 nurse practitioners, 30 residents, 3 fellows, and 45 staff members and are committed to interprofessional, patient-centered models of care. The extended care team also includes integrated behavioral health consultants, social workers, nurse care managers, and population health support teams. Clinical services include adult and children’s primary care, prenatal care, reproductive health services, colposcopy, treatment of opioid use disorder, gender-affirming care, procedure clinics, and obesity management. Founded in 1974, the Department of Family and Community Medicine has graduated 382 family physicians from its residency programs, including many leaders in the field. Nearly half of graduates from the past ten years choose to work in medically underserved areas; a similar number choose academic careers.

Responsibilities will include:

Jefferson Addiction Multidisciplinary Services Sheller Bridge and Hospital to HOME

  • Outpatient addiction medicine (including medications for opioid use disorder, medications for alcohol use disorder, and complex addiction management) and primary care (including outpatient women’s health visits and routine gynecological exams) for patients accessing the Sheller Bridge Program
  • Assume the position of assistant medical director of the Hospital to HOME program and attend weekly and monthly team meetings with other academic medicine and community partners
  • Supervision of addiction medicine fellows at Hospital to HOME and the Sheller Bridge Program
  • Outpatient and home-based addiction and primary care of patients in the Project HOME Hospital to HOME program at the St. Elizabeth’s Recovery House, Beacon House, and other sites as added
  • Teaching of students and residents rotating at the Sheller Bridge Program

Jefferson Family Medicine Associates

  • Outpatient family medicine including women’s health visits, routine gynecologic exams, and children
  • Competency in office procedures strongly preferred
  • Prenatal care preferred but not required
  • Work closely with a strong interprofessional team (including MA, RNs, behavioral health, social workers, population health specialists) to provide patient-centered care.
  • Establish and maintain a panel of patients, manage chronic health conditions, acute conditions, and preventive care; and work closely with clinical and population health staff to meet quality metrics and care coordination needs
  • Opportunities to teach students and residents in the practice of primary care.
  • Commitment to providing highest quality care equitably across a diverse patient population

Requirements: Qualified applicants will hold a MD or DO degree and be board certified, or eligible, in Family Medicine. Must have or obtain a PA medical license.
