Position Summary:

 

The Registered Nurse Assessment Coordinator (RNAC) provides for the initial assessment and periodic re-evaluation of all residents admitted to Skilled Nursing. Conduct Interdisciplinary Team Care Conference and implement and update resident Plan of Care.

 

 

Essential Duties:

  • Ensure accuracy of all sections of Multiple Data Set (MDS) coding to maximize company reimbursement consistent with the levels of treatment delivered and required for appropriate patient / resident care (CMI).
  • Ensure accuracy of clinical paperwork (assessment and accompanying documentation) to maximize company reimbursement consistent with the levels of treatment delivered and required for appropriate patient / resident care.
  • Nursing Educator: – Serve as a primary trainer to the nursing department to ensure documentation is accurate, timely, and thorough. This requires high levels of in-person interfacing with both residents and employees.
  • Nurse Management: Serves as a key member of the nursing leadership team to clinically manage the unit in order to maximize patient-centered outcomes. This includes engagement in leadership standups and influence within interdisciplinary teams.
  • Case management w/ managed care (maximize therapy and nursing services provided to residents in order to maximize patient-centered outcomes)
  • Manage clinical and financial record keeping and dissemination of these records to appropriate payers. This includes overseeing UMR reviews and any appeals processes.
  • Take joint ownership of care conferences with social workers
  • Perform other duties as required.

Related Experience:

  • Minimum 2 years related experience
  • Knowledge of state and federal regulations and requirements under Omnibus Budget Reconciliation Act (OBRA).

 

Education or Degree Requirement:

  • Graduate of accredited School of Nursing
  • Current license with the State of Pennsylvania for Registered Nurse

 

Licenses, Certifications, Registrations:

  • Registered Nurse – licensed in the State of Pennsylvania not required but highly desired
  • AANAC certification (desired, must have within 12 months of employment)
  • Must maintain current CPR Certification.
