Part Time and Per Diem Licensed Practical Nurses/ LPNs

- Must be willing to provide in-home nursing care services (wound care, infusion services, medication education) to adults in the home setting.
- Responsible for documenting in the electronic record all visits with detailed nursing notes to coincide with care provided.
- Provide oversight for non-skilled staff members providing direct care to recipients in the home setting.
- Role is primarily field based with minimal time spent in the office, schedule is flexible.
- Must be a graduate from an accredited LPN/LVN School of Nursing.
- Must be licensed in the state of Pennsylvania, as a LPN.
- One, or more, years of experience, in a community/home health agency or in a home setting, is preferred but not required.
- Must be detail oriented
- Must have a criminal background check.
- Must have current CPR certification. Online certification is not acceptable.
- Physical and TB test required.
- Must have a valid driver's license with current active auto insurance.
