Job Summary:The LPN is responsible for administering the care plan and the patient’s health and wellness as indicated by the care plan. Assures compliance with nursing/clinical standards, federal, state, and local government regulations, contract/ payor requirements and company policies. This role will treat patients, when appropriate and as needed for skilled nursing services.Essential Duties and Responsibilities:
- Follows the appropriate care plan in collaboration with Registry Nurse and Director of Nursing, patient/client/caregivers, the patient’s physician, and other care team members as appropriate and according to the agency policy.
- Performs duties according to professional standards as delegated by a Registered Nurse supervisor.
- Delegates nursing duties to caregivers to safely meet the patient’s needs within the LVN/LPN scope of practice according to local, state, federal, and licensing board regulations.
- Performs on-site orientation, supervision visits, skill checks and annual performances of home care aides in compliance with federal, state, and applicable accrediting body regulations and AHC policies.
- Provides on-going instructions to home care aides during field visits including reinforcement of standard precautions, client respect, HIPAA, and safety. Demonstrates a desire to set and meet objectives and find increasingly efficient ways to perform tasks.
- Develop and review personal care plans from all contract/payor sources, orienting aides in following the care plan as authorized by the contract/payor source nursing staff.
- Documents services rendered and changes in client conditions and/or family and home situation.
- Communicates with contract case managers, coordination staff, and client’s legal representatives any concerns or issues regarding client status. Participates in team/case conferences.
- Performs other duties as assigned.
- Travel and home visits within the community reach of the agency will be required of this role.
Knowledge, Skills, and Abilities:
- Completes work with accuracy and within time frames.
- Requires minimal supervision and is self-directed.
- Adheres to agency infection control and safety policies, including education, reporting, and practice implementation specific to job position.
Qualifications
- Current and unencumbered license to practice as a practical/vocational nurse specific to that state the employee is assigned to work by the Company.
- Current CPR certification.
- Infectious disease clearance for direct patient care
- Reliable transportation and a valid driver’s license is required to perform duties of this role
Work Environment:
- Frequent degree of travel required.
- Ability to freely bend, kneel, sit and stand for potentially extended periods and the ability to lift a minimum of 50 pounds

These services include (but are not limited to) assistance with self-administration of medications, personal care such as assistance with personal hygiene, dressing, and feeding, homemaking such as assistance with household tasks housekeeping, shopping, meal planning and preparation, and transportation, companionship, respite care such as assistance and support provided to the family; and other non-skilled services. This training includes information regarding personal hygiene, safe transfer techniques, infection control, and basic nutrition. All direct care workers of our company also receive supervised practical training before having direct contact with a consumer or verification of prior experience.
