PROGRAM: IPP Case Management
JOB OVERVIEW:
The full-time IPP Case Manager reports to the IPP Case Management Supervisor. The Case Manager works directly with individuals who are court-stipulated to participate in treatment services with oversight from the Adult Probation and Parole departments. The primary responsibilities of this role are to ensure that services are provided to participants and documented according to DAP, CBH, Forensic Services, and PHMC requirements. The Case Manager will provide support to participants while working with all levels of management within the treatment program, Forensic Services, and the recovery community.
RESPONSIBILITIES:
The duties of this role include but are not limited to the following.
- Provide case management services to a chemically dependent forensic population, up to a caseload of 45 participants.
- Establish and maintain service productivity goals at minimum of designated service units per week.
- Document service provision within 24 hours of service completion to individuals assigned.
- Participate in monthly team meetings and scheduled case conferencing with probation meeting reporting requirements.
- Participation in biweekly supervision meetings.
- Collaborate and attend meetings with probation/parole, treatment providers, and other collaborating agencies.
- Complete discharge summaries within the timeframes specified by the program.
- Assist individuals to link to recovery support services including PRO-ACT Recovery Center, Access to Recovery, faith-based institutions, mental health services, and other identified community supports.
- Write weekly schedules that detail the activities to be completed for each week.
- Maintain participant records in accordance with agency fee-for-service standards, managed care, and licensing requirements.
- Prioritize assisting participants to enroll in treatment, physical health services, DPA, vocational services, and educational services.
- Identify, coordinate, and link community resources to participants to promote accessibility to recovery-oriented community services.
- Coordinate and communicate with other programs, within and outside the agency, to foster a collaborative team approach.
- Spend an average of 80% of the workweek in the community for participant appointments and provider meetings.

REQUIREMENTS:
Must meet one of the following:
- Associate degree in social services, sociology, psychology, criminal justice, or social work and 2+ years of experience in human services, including 1+ years of direct participant contact in an addiction recovery, mental health, or behavioral health setting
- Bachelor’s degree in sociology, social work, psychology, anthropology, political science, history, criminal justice, counseling or education and 1+ years of experience involving direct participant contact in an addiction recovery, mental health, or behavioral health setting
PREFERRED EXPERIENCE:
- Experience in direct case management services with a chemically dependent criminal justice population
- Experience in fee-for-service delivery
SALARY GRADE: 15
