GENERAL RESPONSIBILITIES
Assist program participants in accessing housing resources and/or empowering them in their housing stability process.
ESSENTIAL DUTIES
- Identify new and existing patients to determine program eligibility by reviewing patient registration information, social determinant screenings, and income. Document eligibility appropriately.
- Maintain a caseload of eligible participants and work with them to eliminate barriers to housing stability. Meetings may take place in the clinic, community or at patients' homes.
- Complete a housing stability plan with each program participant.
- Assist participants as needed through each step of the housing process, including housing search, application, move in, and ongoing support to maintain housing, as needed.
- Be an advocate for the participant, as needed, and with written authorization in the form of a valid Release of Information (ROI).
- Maintain collaborative linkages with other programs that serve unhoused and unstably housed, including existing local social services agencies, shelter providers, Our Path Home CONNECT, local housing authorities and other appropriate providers.
- Assist participants in the completion of paperwork associated with securing and maintaining housing.
- Complete encounter documentation for all work done in a timely manner and as indicated by standard documented operating procedures.
- Facilitate warm handoffs and/or referrals to other clinic staff such as Outreach, clinical divisions and assist participants in accessing other needed specialty services in the community.
- Acts as a housing resource for clinic staff.
MINIMUM QUALIFICATIONS
- Associate degree in a health or social services related field or 2 years equivalent experience.
- Clearance through the Idaho Department of Health and Welfare Criminal History background check.
- Demonstrated willingness and desire to work with low-income and unhoused populations.
- Demonstrate an understanding of the barriers for people with behavioral health and substance misuse problems.
- A working knowledge of Boise area social service and housing resources.
- Ability to work as a team member.
- Good verbal and written communication skills.
- Reliable and courteous.
PREFERRED QUALIFICATIONS
- Experience working with unhoused patients.
- Experience working in a medical setting.
- Experience working as part of a multidisciplinary team.
