JOB DESCRIPTION:
DVC Bi-lingual Case Manager-Evaluator reports to the Senior Case Management Supervisor and indirectly to the Director of Integrated Case Management. Provide case management and behavioral assessment services primarily for identified DVC participants. This individual works directly with participants having any forensic involvement in addition to any combination of the following: substance use, mental health, and are a participant under the supervision of Domestic Violence Diversion Court. Valid driver license required.
Responsibilities: are to include, but are not limited to:
Case Manager specific duties:
- Ensure case management support and referrals for Domestic Violence Court ordered participants on an as needed basis.
- Case Managers will serve as liaison between the treatment facility and DVC legal team (including DVC judge, attorneys, and FJD court coordinator) acting as support to streamline communication of participants treatment progress and legal status/requirements.
- Case Manager will review all court reports from DVC providers for accuracy and ensure they are received by the court as scheduled.
- Submit and maintain timely and accurate data in Client Registry System ensuring proper data collection related to DVC program.
- Assist during DVC progress listings such as: orientating new participants, addressing participant concerns, and verifying participant information.
- Complete progress reports for submission to court.
- Active participation in team meetings, scheduled provider case conferencing, trainings as assigned.
- Active participation in individual supervisions case reviews, work group, and scheduled team building events as assigned.
- Collaboration with identified court personnel, D&A treatment providers and other assigned agencies within and outside the agency to foster a collaborative team approach, as identified.
- Identify, coordinate, and assist individuals to link to community resources/ recovery support services including Pro-ACT recovery center, Access to Recovery, faith-based institutions, mental health services and other identified community supports.
- Prioritize assisting participants to enroll in treatment, physical health services, DPA, vocational services, and educational services.
- Flexible work schedules available with supervisor approval if needed to provide participant services. Adherence to documentation and approval guidelines required.
- Aid other Forensic Services initiatives through case monitoring to meet departmental requirements
- Case Managers will perform all other duties as assigned.
Evaluator specific duties:
- Supplemental responsibility to conduct behavioral health screening (and as requested full assessments) for eligible participants involved in Domestic Violence Court.
- Be available and accessible for emergency screenings required by the DVC legal partners.
- Complete the written summary of the screening including the recommendation which will be distributed to the management and the DVC legal team.
- Complete warm handoffs to treatment upon screening of participants in DVC program.
- Weekly schedules which detail activities for each week to be completed in outlook feature.
- Maintain participant records in accordance with agency fee for service standards, managed care, and licensing requirements.
- Monitor appropriateness of participants’ level of care and advocate for change if needed.
- Coordinate participants’ legal status with court room staff.

- Must be able to speak fluently in Spanish
EXPERIENCE:
- Experience in direct case management services with the chemically dependent criminal justice population a plus.
- Experience with domestic violence populations a plus.
- Experience in fee for service delivery and service note composition a plus.
EDUCATION:
- Minimum Bachelor’s degree in related field and one (1) year of direct participant contact.
- Addiction Recovery, Mental Health, Behavioral Health setting, preferred.
