JOB OVERVIEW:
Joseph J. Peters Institute (JJPI) seeks a Case Manager for its Survivor Services program. This person will ensure compliance with stakeholder targets; monitor and manage databases, tracking, and data systems; assist with case assignments; complete medical records requests; audit charts; and conduct trainings. This position reports to the Case Management Coordinator and Operations Manager.
RESPONSIBILITIES:
- Maintain the highest quality case management services to patients at JJPI.
- Develop and implement innovative techniques to serve the clients more effectively.
- Enter all outpatient facility participants into databases upon intake.
- Scan, enter, and track all documentation throughout the course of the participant’s treatment.
- Monitor and manage databases, tracking systems for evaluations and outpatient data, assigned cases, and discharged participants.
- Act as a liaison between participants and clinical staff.
- Communicate with participants, other agencies, courts, and stakeholders regarding participants’ assignment timelines, progress, attendance, and compliance.
- Ensure compliance with stakeholder targets and uphold HIPPA guidelines within all communications.
- Assist in ongoing quality management of Survivor Services. Specifically, provide ongoing chart audit, treatment plan tracking, and compliance with progress note completion parameters.
- Compile and track medical records requests and invoices for other agencies and stakeholders.
- Assist with insurance verifications and service approvals when the Quality Assurance representative is out for an extended time.
- Assist with office assignments and technological issues when needed.

Preferred: Working knowledge of PowerPoint, Excel, Access, and SPSS
EDUCATION: Bachelor’s degree in a field related to clinical work
EXPERIENCE:
- Two to three years of experience in the field
- Experience with human services systems
- Knowledge of resources available to staff and clients that enhance, coordinate, and complement the services provided at JJPI
SALARY GRADE: 15
