PROGRAM OVERVIEW:


Pathways to Recovery (PTR) is Philadelphia’s short-term co-occurring disorder treatment partial hospitalization program. The program is funded by Community Behavioral Health (CBH) and Behavioral Health Special Initiative (BHSI) for Philadelphia residents with co-occurring substance use and mental health challenges. Our program works with participants 20 hours per week, for up to nine weeks. We provide a menu of treatment services available each day including individual, group, and family therapy; medication-assisted treatment and medication management with our psychiatrist; health care coordination and full-time nursing support; full-time case management; and full-time peer recovery specialist support.


JOB OVERVIEW: We are seeking a full-time Case Manager to provide case management and psychoeducation to PTR participants.


RESPONSIBILITIES:



  • Perform client needs assessments based on intake materials and client interviews.

  • Provide case management/care coordination and advocacy by assisting clients in negotiating social services, providing follow-up and monitoring support. Reassess client needs as appropriate.

  • Maintain contact with the client’s therapist and other supports as needed.

  • Complete a continuing care plan with each client two weeks prior to discharge.

  • Maintain accurate, timely records of time spent with each client, problem areas, problem resolution, etc. Ensure that charts meet all applicable standards.

  • Coordinate and monitor client use of TransPasses. Facilitate Logisticare transportation. Transport clients to social services and appointments as needed.

  • Cofacilitate specialized client groups that are focused on conflict resolution, life skills, etc.

  • Identify housing options for clients and coordinate admission to housing.

  • Provide input to the clinical team.

  • Attend weekly supervision and other required meetings. Participate in staff development as required.

  • Perform other duties as assigned.


SKILLS:



  • Expertise in case management and client advocacy

  • Ability to work with individuals who are struggling with addiction in critical and emergent situations

  • Sensitivity to cultural and economic differences

  • Thorough knowledge of social services and resources available in Philadelphia

  • Understanding of medical and psychosocial issues related to addiction


QUALIFICATIONS:



  • Must meet one of the following:

    • Bachelor’s degree with 3+ years of experience in social/human services

    • Master’s degree with 1+ years of experience



  • Valid driver’s license and ability to drive a 15-passenger van
